Mailchimp Integration in NewLifestyle position bottom-extra

I am trying to figure out how to get mailchimp to capture the email address entered in the bottom-extra field on the front page at www.therapymaps.com/ACWC333

I would be very grateful for any help!

I'd take the mailchimp form code and paste it in using the editors html editor

Removing the existing email capture box
