Media manager only allows uploads of alphanumeric file names. So, the previous use of the + sign to create a space doesn't work anymore.
You can see the site on www. ecfcmma.com

For example:
I uploaded file: East+Coast+Fighting+Championship.jpg
When I uploaded to media manager it renamed it to: EastCoastFightingChampionship.jpg

So, although "File name separator character" in image source settings is set to the default of + , it doesn't affect change since it's being removed within joomla.

Any suggestions?

Try using underscore character instead

file_name_separator

That will work for you

As a nice bonus if you use the JCE editor it will strip the underscore for the alt tag if added to an article

I guess I should have tried that. I guess the underscore is allowed but plus is not. Weird!
