I am searching for an intuitive form designer for Joomla 3 and that will specifically work well in terms of aesthetic integration with Venture.

I need to design an application form for a school and have a few requirements and was hoping someone could recommend a solution based on experience. I have listed the requirements below.

1. Multi Page form as the form is quite large
2. Ability to have routing based on response, I.E. display different set of questions if answer is yes as apposed to no
3. Form field validation based on character type, count and format
4. Allow for uploading of supporting documents/pdf files or images
5. Allow for acknowledgements by ticking etc
6. Form security like captcha and questions obviously

The problem might be the "aesthetic integration with Venture."

thanks for the quick response, looking at rsforms now. If you could ask the team if they would be able to assist with the integration bit, just styling and very basic styling just so it does not look like an add on would be awesome. you could supply me with costing for this if possible please.
